Tree Replacement Form

I/We, the undersigned, do hereby agree to abide by the provisions set forth in Chapter 48-19 to 48-37 in the Code of Ordinances for the City of Tomah, WI, including those of the Wisconsin Statutes, Wisconsin Administrative Code or other materials which are incorporated by reference.

I/We understand that the Director of Parks and Recreation and the Director of Public Works, under the direction of the Parks and Recreation Commission, shall superintend, regulate, and encourage the planting, car and removal of all trees and shrubs on all boulevards and in any other public place in the City.

This permit is not valid unless signed by the property owner and the Director of Parks and Recreation.
